• A valid Master Card or Visa credit or debit card for the application fee of $150.00 which is nonrefundable.
• Name
• Address
• State, year and License Number of original licensure by examination
• Name under which originally licensed
• State of current licensure and License Number
• Name and location of your basic School of Nursing
• Year Graduated from your basic School of Nursing
• All US States/Possessions/CN Provinces in which you have EVER been licensed with License Number and dates License held. (Note: Canadian educated applicants must have successfully passed NCLEX. Clickhere to apply online for NCLEX.)
• All states in which you are currently practicing nursing (including telenursing)
• Last two (2) employers. Name, location of where nursing license required (either for paid position/consulting/self-employed/volunteer work) along with dates of employment and position held.
• Date of birth
• For security reasons, mother's maiden name (Last Name only)
