Controlled Substances Reporting System (CSRS)

Enrollment and Utilization of the CSRS

Statute § 90-113.74A stipulates that within 30 days after obtaining an initial or renewal license that confers the authority to prescribe a controlled substance for the purpose of providing medical care for a patient, the licensee shall demonstrate to the satisfaction of the licensing board that he or she is registered for access to the controlled substances reporting system. A violation of this section may constitute cause for the licensing board having jurisdiction over the licensee to suspend or revoke the license. (2016-94, s. 12F.7(c).

NC Board of Nursing through collaboration with NCDHHS is providing a streamlined registration process to the NC Controlled Substances Reporting System (CSRS). The system features the following:

  • APRN’s with prescriptive authority access the CSRS online registration via the NCBON Nurse Gateway
  • Licensee information currently stored by NCBON will prefill the application
  • Other information not stored by NCBON but required by CSRS, such as DEA number will be entered by the licensee
  • This additional information will be combined with the information on record with the NCBON and sent electronically to the CSRS office for registration. Prior to transmitting the data the prescriber will sign the privacy statement and give consent for the transmission



Submitting the CSRS Application

Please note that the CSRS registration is accessed within the NCBON Nurse Gateway and you will first be directed there from the CSRS registration link below.  The Nurse Gateway provides Nurses and Nurse Aide II's a single portal to manage licenses and listings; allows access to printable documents, change of address along with many more features. The Nurse Gateway is used to submit all license and listing applications including Advance Practice approvals.

The completed CSRS application will go directly to the NC DHHS Drug Control Unit. Accordingly, any questions about the application once submitted should be addressed by email to or by phone at (919) 733-1765.
