Nurse Practitioner - Add a Physician:

Provides the ability to add new supervising physician(s).

  • $100 per physician for non-volunteer status or,
  • $20 per physician for volunteer status (non-paid position)

FEES ARE NON-REFUNDABLE

Note: Please be sure you indicate the correct status of your position as either volunteer or non-volunteer for each approval.

  • Credit or debit card; MasterCard or Visa only 
  • Electronic Check

You may print your confirmation page following successful completion and payment. This document serves as your official receipt.

Additional Information:

Upon FINAL approval to practice with the new supervising physician, the approval letter and certificate will be emailed to the NP. The approval can also be verified on the Board's website by selecting Verify License. The change or additional practice information must be posted on the website or the approval certificate & letter received before the NP starts their new job.
