Nurse Aide II

Listing Requirements
  • Must be listed as a Nurse Aide I on the North Carolina Division of Health Service Regulation-Nurse Aide I Registry with no substantiated findings of abuse, neglect or misappropriation of property;
  • Must have successfully completed a Nurse Aide II program;
  • Must submit application for Initial Listing within 30 business days following completion of a Nurse Aide II program; or
  • For an Registered Nurse or Licensed Practical Nurse nursing student, application must be submitted within 30 business days following completion of course work equivalent to above Nurse Aide II requirements.
Military Applicants

U.S. Military applicants must be listed as a Nurse Aide I with the Division of Health Service Regulation-Nurse Aide I Registry and must have been discharged within the past two years. Submit the following documents to the Nurse Aide II Registry for review:

Navy Hospital Corpsman

  • Copy of current Nurse Aide I listing; 
  • Copy of official military documentation indicating completion of Basic Corpsman class “A”; 
  • Copy of military documentation indicating rank of Third Class or higher; 
  • Copy of military documentation indicating discharge date (within past two years) or current enlisted status

Army

  • Copy of current Nurse Aide I listing;
  • Copy of official military documentation indicating completion of 68W10 – Health Care Specialist program;
  • Copy of military documentation indicating discharge date (within past two years) or current enlisted status

Air Force

  • Copy of current Nurse Aide I listing;
  • Copy of official military documentation indicating completion of the Air Force Medical Service Technician Program;
  • Copy of military documentation indicating discharge date (within past two years) or current enlisted status

Initial Listing

All Nurse Aide II Initial Listing applications must be submitted within 30 business days following completion of a Nurse Aide II program. If your application is not submitted within 30 business days, you will be required to retake the full Nurse Aide II program.

A Nurse Aide II listing is non-transferable (to or from North Carolina).

Listing Requirements

  • You must be listed as a Nurse Aide I on the North Carolina Division of Health Service Regulation-Nurse Aide I Registry with no substantiated findings of abuse, neglect or misappropriation of property;
  • You have successfully completed a Nurse Aide II program; 
  • You must submit an application within 30 business days following completion of a Nurse Aide II program; or
  • For a Registered Nurse or Licensed Practical Nurse nursing student, you must submit an application within 30 business days following completion of course work equivalent to above Nurse Aide II requirements.

How to submit...

  1. Access the Nurse Portal to complete the Nurse Aide II Initial Listing application
  2. Create a Username and Password to access your information and applications. If you have already created a Username and Password, you will only need to log in to your profile.
  3. Select 'NAII Application' in the Licenses/Certificates dropdown
  4. Complete application

      • The confirmation number provided on the applicant confirmation page will be required for payment.
    • Business checks must be made out to the North Carolina Board of Nursing, and we request that candidates provide a copy of the confirmation page to reduce processing delays. Mail your business check to North Carolina Board of Nursing, P.O. Box 2129 Raleigh NC 27602-2129.
    • Once your payment has been received, allow five banking days for your payment to clear, and then your application will be processed.
    • You will not be declared eligible until the Board of Nursing receives the required documents and fees.

The Application Confirmation which serves as your official receipt will be available on the Nurse Portal profile upon successful completion of the application process. To access, select the appropriate tile under the 'Licenses/Approval to Practice/Certifications' tab on the Home screen, then select the 'Files' tab on the License Information screen.

If your email address is listed with the North Carolina Board of Nursing, you will receive a confirmation via email verifying receipt of your online application.

Upon successful completion of the online application, you may check your application status via Nurse Portal. Licensure information can be verified on the Board's website. The Board of Nursing’s electronic database serves the primary source of licensure information for nurses in North Carolina.

Program Completion Verification

Nurse Aide II students must submit an application online within 30 business days following successful completion of a Nurse Aide II program.

Nursing students must submit an application online within 30 business days following successful completion of course work equivalent in content and clinical hours to that required for a Nurse Aide II.

Only the Registered Nurse-Nurse Aide II Instructor (or the officially designated person) of the Board approved North Carolina Nurse Aide II program will have access to this system. It is password protected.

The Nurse Aide II Program Completion Verification must be completed by the Registered Nurse-Nurse Aide II Instructor for students of a Nurse Aide II program or the Nursing Program Director for nursing students currently enrolled in Board of Nursing approved nursing programs.

Registered Nurse Instructor or Nursing Program Director

Must certify the following:

  • Student successfully completed all requirements for listing as a Nurse Aide II;
  • Date of student's successful completion of Nurse Aide II program or Board of Nursing approved nursing program

Program Completion Verification Requirements:

  • A valid unencumbered Registered Nurse North Carolina license or Compact license
  • First and Last Name of student
  • Date of Birth of student

After the submission of the students who completed the program successfully, the student may then apply online for listing as a Nurse Aide II. The student must submit the application using the same identifying information as submitted in the Nurse Aide II Program Completion Verification.

Licensure information can be verified on the Board's website. The Board of Nursing’s electronic database serves the primary source of licensure information for nurses in North Carolina.

 

The North Carolina Board of Nursing will only provide the program’s username and password to the officially designated person at the program. If you need to update the officially designated person for your program, contact the North Carolina Board of Nursing Education Department using the Email Us Form for a new password.

Program Director Portal

Listing Renewal

If your Nurse Aide II listing expired in the past 24 months, you are required to successfully complete the competency evaluation portion of the Nurse Aide II Program to be eligible for renewal.

If your Nurse Aide II listing expired in excess of 24 months, you are required to successfully complete a Nurse Aide II Program to be eligible for renewal.

Allow 5-7 days before your expiration date to complete this process.

Online submission of your renewal application must not be construed as automatic renewal of your listing. The North Carolina Board of Nursing must verify that you have met all the conditions for renewal before your listing is renewed.

Renewal Requirements

  • You must be listed as a Nurse Aide I on the NC Division of Health Service Regulation-Nurse Aide I Registry with no substantiated findings of abuse, neglect or misappropriation of property; 
  • You must have performed nursing care activities for monetary compensation within the past 24 months for at least 8 hours under the direct supervision of a Registered Nurse; 
  • You must submit an application on or before the last day of your birth month

2-Step Process

  1. Nurse Aide II must submit the online renewal application; and
  2. The Registered Nurse Supervisor must complete the online Nurse Aide II Employment Verification

  1. Access the Nurse Portal to complete the Nurse Aide II Listing Renewal application
  2. Select your listing
    1. Your listing includes your listing type (NA II Listing), listing number, expiration date, and listing status.
  3. Select 'Renewal/Reinstatement' in the Applications tab
  4. Complete application

Employment Verification

A Registered Nurse from the same agency where the Nurse Aide II is/was employed must complete the online Employment Verification and validate that the Nurse Aide has performed nursing care activities for monetary compensation within the past 24 months for at least 8 hours under the direct supervision of a Registered Nurse.

Employment Verification Requirements

  • Your valid unencumbered Registered Nurse license certification number
  • The Nurse Aide II listing number
