Request Name or Address Change

Completion will change your name or address in the North Carolina Board of Nursing records.

In accordance with the Nurse Licensure Compact, change of address may affect the practice privilege for your North Carolina nursing license.

Submission

How to submit...

  1. Access the Nurse Portal to Request a Name Change or an Address Change
  2. In the top right-hand corner, select Profile Name
  3. Select 'Request Name Change' or 'Request Address Change' dropdown
  4. Complete application
Upload the legal documentation to your nurse portal supporting your name change to the North Carolina Board of Nursing.

*Examples of supporting legal documentation include:

  • Social Security Card
  • Change of name certificate as issued by a court (Required for any first name change)
  • Birth Certificate
  • Marriage License
  • Divorce document reflecting name change
  • Passport
  • Immigration documents reflecting name change
  • Driver's License
  • Voter Registration (for address only)
  • Federal Income Tax Return (for address only)
Mail, email, or fax your supporting documentation

To complete the name change, you must upload a photocopy of the legal document that reflects the new legal name to your nurse portal.
