Initial Approval to Practice

This application is for the Registered Nurse applying for initial approval to practice as a Certified Nurse Midwife.
The Certified Nurse Midwife shall not practice until notification of approval to practice has been received from the Board of Nursing. The approval letter and certificate will be emailed to you upon final approval to practice.
You will need the following to apply:
- A permanent North Carolina Registered Nurse (RN) license or another Compact State Registered Nurse license valid for practice in North Carolina (Compact license information must first be submitted on the "Compact State Application")
- Physician Name or Physician License Number and Physician Email Address
- Practice Information (Practice Name, Practice Address, Office Phone, Email)
In addition to the online application, additional documents must be mailed to the North Carolina Board of Nursing. These may include:
- Identification Document (Form available upon submission of application)
- Official transcript and/or Midwifery education documents
- Copy of American Midwifery Certification Board (AMCB) Certification
- Licensure Biography (verification) of Certified Nurse Midwifery license/practice in other states, if applicable. (Form available upon submission of application)
- Disclosure of any malpractice claims to be reported and submitted to the Board of Nursing on the Claims Information Form , if applicable.

This application is for the Certified Nurse Midwife applying for renewal of approval to practice.
You are required to renew your approval(s) to practice by the last day of your birth month each year. If you do not renew by the last day of your birth month, your approval to practice is automatically placed on inactive status at midnight on the last day of the expiration month.
A Certified Nurse Midwife with an inactive approval to practice status shall not practice as a Certified Nurse Midwife. Prior to practicing, you will be required to reapply for approval to practice and will need to complete the Approval to Practice Reinstatement application.
Maintaining certification as a Certified Nurse Midwife through American Midwifery Certification Board (AMCB) is also required. If you have renewed or updated your Certified Nurse Midwife Certification since your last renewal, contact AMCB to request verification be sent to the North Carolina Board of Nursing.
Reinstate Approval to Practice
Fee Information: $50.00, and $5.00 (Administrative Fee)

This application is for the Certified Nurse Midwife applying for reinstatement of approval to practice.
The Certified Nurse Midwife shall not practice until notification of approval to practice has been received from the Board of Nursing. The approval letter and certificate will be emailed to you upon final approval to practice.
Add Physician
This application is for the Certified Nurse Midwife adding a new supervising Physician.
The Certified Nurse Midwife shall not practice until notification of approval to practice has been received from the Board of Nursing. The approval letter and certificate will be emailed to you upon final approval to practice.
Physician Maintenance
Fee Information: There is no fee for this process
This application is for the Certified Nurse Midwife to:
- submit additional practice sites to your currently approved practice(s);
- update the practice information (Practice Name, Practice Address, Office Phone, Email) for an existing approved Physician; or
- remove an existing approved supervising Physician
Request Inactive Status
Fee Information: There is no fee for this process
This application is for the Certified Nurse Midwife requesting inactive status for all supervising Physicians.
Remove a single existing approved supervising Physician via the Physician Maintenance application.
Update Compact Registered Nurse License for Certified Nurse Midwife - Expiration Date
Fee Information: There is no fee for this process
This application is for the Certified Nurse Midwife to change the Registered Nurse information in the North Carolina Board of Nursing's records for another Compact State license. Once the application is submitted, we will verify the information with the Compact State.
